The two things everyone asks about
Both of these get asked in the first ten minutes of every conversation, so here they are in full.
How the wait estimate is worked out
It's a rolling average of the consultations that doctor has actually finished today — not the booking interval, and not a number someone typed in during setup. Early in the day, before there are enough completed consultations to average, it falls back to a flat fifteen minutes per patient and says so. The effect is that a slow morning shows up as a longer estimate within a couple of patients, instead of the schedule quietly lying until someone complains.
When a patient actually gets a message
Two triggers, both threshold-based. One fires once when a patient's position drops to five or fewer — the "start heading over" message. The other fires when the queue shifts under them by three or more places, which is the case where an estimate they read twenty minutes ago is now wrong. A one-place shift sends nothing. The point is that a message from Dr.incity always means something changed, so people keep reading them.

What it deliberately doesn't do
A short list, published on purpose. Every one of these is a decision, not an omission we're hoping you don't notice.
It is not an EMR
No clinical notes, no diagnoses, no prescriptions. The queue knows an appointment exists and who it belongs to. Records are a separate, later product.
It does not take payments
Clinics collect exactly the way they do today. No card details ever touch Dr.incity.
It does not check patients in for them
Arrival is confirmed at the desk by reception, on purpose. A phone that thinks it is at the clinic is not the same as a person who is.
It does not span clinics yet
A queue belongs to one clinic and one doctor. A delay at one clinic does not propagate to another — that is on the roadmap, not in the product.
